Membership Cancellation Request - Performance Training Centre

Performance Training Centre Membership Cancellation/Freeze Request form.

Annual memberships may be cancelled with no less than 30 days written notice before next payment date, subject to a $100 cancellation fee, except as otherwise set out in the Membership Contract.

Continuous Monthly memberships can be cancelled with no less than 30 days prior written notice before next payment date. Member expressly acknowledges that: (i) all payments made prior to the effective date of cancellation are absolutely non-refundable; and (ii) Fees will not be prorated or refunded for cancellations that take effect mid-month or mid-year.
